Lifestyle Swim School – 2024 Application and Waiver Form
Submission of this online Registration Form and Liability Release Form, constitutes acceptance of the terms and conditions of registering your child / children with the Lifestyle Swim School Programs. If you are enrolling three or four children please fill the form out two times.
For your convenience we have placed a copy of the 2024 TBA schedule below the registration form, so that you can refer it.
To ensure your child’s spot in class, all fees and paper work must be completed before child will be placed on the class roster. Once you have competed sending this registration/liability release form, please proceed to the Payment link. Your child will be placed on the class roster once payment is received.
Liability Release Form: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify and hold harmless the Lifestyle Swim School, Inc. their officers, officials, agents and/employees, other participants, sponsoring agencies, sponsors, advertisers, Florida Atlantic University and The Florida Atlantic University Board of Trustees, with respect to any and all injury, disability, death or loss of damage to person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by the law. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others and some full responsibility for my participation of myself and child.
My / Our child (children) has been examined by a certified physician within the last year and is physically fit to participate in swim lessons. He or she is free from any contagious disease.
I / We understand missed classes may not be carried over the next session and that there are no make-up classes for classes that you missed. I understand that the Lifestyle Swim School is unable to grant refunds. We reserve the right to: accept only those families who abide by our patient child-centered philosophy; to expel those families that do not abide by the philosophy, fail to follow rules, use foul language, or are disrespectful to teachers, staff or fellow classmates. Writing your name on the below form implies acceptance of the above liability release form and policies.
